Please send the completed Registration Form to the Conference Secretariat via e-mail, fax or regular mail. You can download the form by clicking the link at the bottom of this page. NOTE! Credit card payments cannot be processed without an authorized signature. Thus, those who would like to pay by credit card, are kindly requested to send a filled in registration form with an authorized signature preferably by regular mail (or fax).

LETTER OF INVITATION
If an official invitation is necessary for you to obtain a visa to attend the meeting, you must either submit an abstract or register for the meeting. For security purposes, letters will not be sent to any attendee that does not meet these requirements. NOTE: Invitation letters will not be processed until you have registered and paid for the registration fees. Request should be addressed to the Conference Secretariat specifying the necessary details.

** In order to receive the reduced fee, students must submit a letter of support from their academic supervisor, confirming their student status. Students are university medical students and residents under 30.
*** One exhibitor per booth free of charge, and maximum further two persons with reduced fee.

Registration fee includes attendance to all scientific sessions, conference official documentation, one conference bag with complete congress materials, book of abstracts, coffee and tea during breaks, admission to the Get-together Party on 23 May 2009 and lunches on 24 and 25 May 2009. Registration fee does not include lunch on Saturday 23 May 2009 (for tutorial participants only), ticket for the gala dinner.

Accompanying persons’ registration fee covers admission to the Get-together Party on 23 May, Saturday and furthermore one gala diner in the national restaurant “Lido” on 25 May, Monday.

PLEASE NOTE: Accompanying persons will not be admitted to the scientific sessions.

PAYMENT PROCESS
Letter of Confirmation and Proforma Invoice

NOTE: You are kindly required not to begin any payment process before receiving an official confirmation of the registration with title ‘Letter of Confirmation and Pro-forma Invoice’. Please check this letter of confirmation carefully to confirm that all the items you have requested are included. Based on this confirmation, you can start the payment procedure. The letter of confirmation must be presented upon registration at the beginning of the conference.

Payment (registration and accommodation fees, charges for tutorials and tourist tours) should be made by bank transfer, in EURO, without charge to the beneficiary, to the following account:

Commissions and bank charges are for your account.

Confirmation of payment will be sent to your e-mail address.

You are requested to indicate the registrant’s name and the conference name (NEURO 2009) in the memo section of the wire transfer.

Please make sure that commissions and bank charges of the ordering customer’s and the beneficiary’s bank are for your account. The Organisers will recover from the participant any loss which may be incurred as a result of converting any currency other than EURO to EURO, or bank charges.

You are requested to indicate the registrant’s name and the conference name (NEURO 2009) in the memo section of the wire transfer.

Receipt
After having received full payment, Conference Secretariat will issue an official Receipt and send it via regular post; furthermore registration will be finalised on receipt of payment.

Congress Secretariat accepts Visa, VisaElectron and MasterCard only. Please download, print and completely fill in the Credit Card Payment Form (pdf document ? MS word document). CANCELLATION POLICY
All cancellations must be sent to the Neurosonology 2009 Conference Organizing agency in writing by the deadlines specified.
Registration fee refund will be made after the event as follows:

• cancellation before 1 March 2009 - 70% refund
• cancellation from 1 March to 30 April 2009 - 30% refund
• cancellation after 1 May 2009 - no refund

Participants who are unable to attend the conference at the last minute are therefore advised to encourage another member of their staff to attend in their place.
